Health Office Information

Welcome to the Elm Drive Health Office!

We strive to provide a safe and healthy environment for all children. Please let us know of important health issues, including any allergies, or changes in your child's health that may affect them at school.

Absences:

Whenever your child is absent, call the health office and send in a note upon return or send an email explaining the absence. The email addresses are at the bottom of the page.

Illness guidelines:

Your child must be fever free for 24 hours, without the use of fever reducing medication, before returning to school. They should not return to school for 24 hours after vomiting, diarrhea, and treatment for contagious illnesses such as strep throat and pink eye.

Medication at school:

All medication, including over the counter, requires a written order from the healthcare provider and signed consent by the parent to be given at school. The medication must be brought to school by an adult. Never send medication to school with your child.

Vision and Hearing Screening:

Students are screened annually in the fall for vision and hearing. If your child does not pass the screening, you will be notified. Please contact me any time during the school year if you have concerns about your child's vision or hearing.


Health Appraisal

All children entering Kindergarten and Second Grade, and those new to the district, are required to have a health appraisal (physical). The school health appraisal form or the pediatrician’s form may be used. A health appraisal done within 12 months of the start of the school year is valid. Please send in a copy by October 15.

Immunizations

NY State Law, Section 2164, requires proof of immunization for school attendance. The following must be given at ages and intervals in accordance with the ACIP (Advisory Committee for Immunization Practices) schedule:

  • Diphtheria/tetanus/pertussis (4-5 doses)- Must receive a dose at age 4 or older
  • Hepatitis B (3 doses)
  • Polio (3-5 doses)- Must receive a dose at age 4 or older
  • Measles/mumps/rubella (2 doses)
  • Varicella (chickenpox) (2 doses)


Please check with your health care provider before the start of school to make sure your child has all the required immunizations for school attendance.

Proof of immunization must be 1 of the following 3 items:

  • Immunization record signed by healthcare provider
  • Immunization registry report (NYSSIS) or county health dept. record
  • Laboratory report of a positive titer proving immunity to the disease(s). For varicella, a note from the healthcare provider that your child had the disease is acceptable.
